ABSTRACT |
Idiopathic stenosis of the upper airway is characterized by heavy inflammatory changes along the respiratory tract accompanied either by laryngeal stenosis or tracheomalacia. The commonest causes include Wegener's granulomatosis with upper airway involvement and relapsing polychondritis. Takayasu's syndrome and Bechet's syndrome are some other citied causes, but these conditions often defy exact differentiation. The authors are reporting 8 recent cases of idiopathic stenosis of the upper airway in order to aid in future recognition and treatment of this condition by attempting a clinical analysis with regard to the clinical courses, laboratory findings, and their respective changes with treatment. There were 2 males and 6 females, showing female predominance with respect to sex distribution, and all patients were above the age of 15. Four cases had been diagnosed as relapsing polychondritis, and in 4 cases Wegener's granulomatosis seemed to be the most probable diagnosis. All patients were treated with prednisolone and in some patients, regimen consisting of cytoxan or dapsone seemed to offer some benefit. Tracheostomy of tracheal fenestration had to be employed in 6 patients in order to establish adequate airway. In remaining 2 patients with milder degree of stenosis, conservative measures had been taken to alleviate obstructive symptoms and they are still under observation.
