ABSTRACT |
The objectives in surgery of chronic otitis media are the elimination of the disease, restoration of the hearing capacity, and maintenance of the normal anatomic contour. The authors clinically analyzed 189 cases of chronic otitis media operated by one operator at Department of Otolaryngology of Jung Ang Gil General Hospital from December 1991 to July 1993. The following results were obtained. 1) Conditions of ossicles; 88 cases(47%) were intact, 101 cases(53%) were destructed. Destroyed ossicles showed the predilection for destruction of malleus & incus. 2) The methods of operation were as follows ; Intact canal wall mastoidectomy was 101 cases(57%), open cavity mastoidectomy 74 cases(39%), modified radical mastoidectomy 2 cases(1%), and radical mastoidectomy 6 cases(3%), respectively. 3) The tympanomastoidectomy with ossiculoplasty was performed using autograft ossicles in 56 cases and autograft cartilage was used in 3 cases. 4) Postoperative hearing gain was better in intact canal wall mastoidectomy cases than open cavity mastoidectomy cases(p<0.05). 5) Postoperative hearing gain at ossiculoplasty was better in short columellization cases than long columellization and M-S assembly cases(p>0.05). 6) The success rate of drum graft was 90%.
