Surgical Management of Cerebrospinal Fluid Rhinorrhea with Free Grafts Under Surgical Microscope

ABSTRACT

Iatrogenic cerebrospinal fluid rhinorrhea may occur after intranasal ethmoidectomy and transsphenoidal approach for removal of the pituitary tumors and is divided into intraoperative and postoperative type according to the time of onset. Recently, we have experienced nine cases of successful repair of cerebrospinal fluid rhinorrhea using fibrin glue-coated fascia or inferior turbinate mucosa under the surgical microscope from 1989 to 1992 at Severance Hospital, Seoul, Korea. Neither recurrent leak nor postoperative complication was associated with the free graft. Our results implicates that repairing CSF rhinorrhea using free grafts is as successful as using local flap and it is simple and effective method in the management of CSF rhinorrhea. The surgical technique is discussed in detail.
