Wide excision of the tumor site and adequate reconstructive surgery are essential to the surgical management for head and neck cancer. Recently, there has been much development in this field, especially various myocutaneous flaps have been widely used for the rapair of the excised area. Five major myocutaneous flaps are currently available to the head and neck surgery : the pectoralis major, the lattisimus dorsi, the sternocleidomastoid, the trapezius and the platysma. Among these flaps, the pectoralis major has been considered excellent one in the view of viability. In our department on May 1984, the Comman do operation was carried out to the two patient with carcinoma of the oral cavity and reconstruction was performed with the pectoralis major myocutaneous flap. The result was good with excellent viability, long pedicle flap, sufficient carotid protection, satisfactory cosmesis and relatively good tongue mobility.
