A Clinical Study on the 18 Cases of Intubation Granuloma of Larynx |

ABSTRACT |
Endotracheal intubation has been used widely for general anesthsia or patency of airway. However, despite of its therapeutic value, several post-traumatic complications such as laryngeal edema, ulceration and granuloma are developed following intubation. Intubation granuloma is localized inflammatory response to mucosal loss caused by endotracheal trauma. For 10 years 1975 to 1984, the authors had experienced 18 cases of intubation granuloma in our department and following results were obtained. 1) Among the total 18 cases, 14 cases had undergone endotracheal anesthesia and 4 cases required endotracheal intubation for patency of airway. Of 14 cases, 2 cases took the operation in the other hospital. Incidence of intubation granuloma over 10 years in 0.05% in patients with endotracheal anesthesia and 0.39% in medical care patients with endotracheal intubation. 2) Age distribution shows mostly in the 21 to 50 years old group(78%), and there is no case under age of 10. The sex ratio of male to female is 5 : 13, predominantly in female. 3) Main symptoms are hoarseness or dysphonia in all patients, foreign body sensation in neck in 14 cases(78%) and dyspnea in 10 cases(56%) of all. 4) Duration of intubation is more than 4 hours in 5 cases of all operated cases(38%) and more than 3 days in 3 cases of medical cases(75%). 5) Composition of used intubation tubes is red rubber tube in almost of all cases except 1 case. 6) The side of lesion is bilateral in 3 cases(17%) and unilateral in 15 cases(83%). The location of mass is vocal process of arytenoid cartilage in 13 cases(72%), subglottic in 2 cases, middle 1/3 of vocal cord in 2 cases. The morphology of mass is pedunclated in 14 cases(78%) and sessile in 4 cases(22%). 7) In treatment, granuloma was removed under endoscopy in all cases.
