ABSTRACT |
The microsurgery in the treatment and diagnosis of laryngeal diseases had been performed by using Lynch's suspension laryngoscope. The introduction of endolaryngeal microsurgery by Kleinsasser in 1965 made more useful approach for laryngeal surgery and diagnosis with self retaining laryngoscope. The microsurgical technique has advanced and gained widespread acceptance. A clinico-statistical analysis was performed on 47 cases of suspension laryngoscopic microsurgery in the Dept. of E.N.T. Jeonbug National Medical School from Oct. 1979 to Jul. 1982. The results were as follows ; 1) Sex distribution was male 1.04 : female 1. 2) Age distribution showed 17 cases (36.2%) in 5th decade, 11 cases (23.4%) in 6th decade, 7 cases (14.9%) in 4th decade, 3 cases (6.4%) in 2nd decade and 3 cases (6.4%) in 3rd decade in order. 3) Chief complaints were hoarseness in 42 cases(89.4%), dyspnea in 9 cases (19.1%), throat discomfort in 9 cases (19.1%) and foreign body sensation in 7 cases (14.9%) in order. 4) Distribution of disease in order of frequency were vocal polyp in 21 cases (44.7%), vocal nodule in 8 cases (17%), Rienke's edema in 6 cases (12.8%), papilloma in 5 cases (10.6%), intubation granuloma in 2 cases (4.3%), leukoplakia in 2 cases (4.3%), squamous cell carcinoma in 2 cases (4.3%), sulcus vocalis in 2 cases (4.3%), verrucous carcinoma in 1 case (2.1%) and traumatic web in 1 case (2.1%) in order. 5) Duration of chief complaints were 17 cases (36.1%) below 6 months, 15 cases (31.9%) from 6 month to 1 year, 13 cases (37.2%) from 1 year to 5 years and 2 cases (5.7%) over 5 years. 6) The involved sites were 18 cases (36%) on both cord, 16 cases (32%) on Rt cord, 13 cases (26%) on Lt cord and each 1 case on anterior commissure, whole larynx and extralarynx in order. 7) Maximum phonation time was measured before and after operation in 25 cases and prolonged in 21 cases (84%). 8) Phonation Quotient was measured before and after operation in 13 cases and decreased in all cases. |
