Clinical Analysis of Middle Ear Disease

ABSTRACT

Chronic otitis media is one of the frequent disease in otolaryngologic field. The treatment of chronic otitis media are difficult management problems. But, in more recent years, the development of aseptic technique, introduction of antibiotics, use of the operation microscope, and application of the research method were much helpful for the treatment of chronic otitis media. At now, many surgical procedures for chronic otitis media have been carried out. Author studied the surgical procedures for chronic otitis media which were done in ENT, department of Han Yang University from Jan. 1973, to Dec. 1979. The subject were 679 cases of surgical treatment. The conclusions were obtained as follows; 1) Sex distributions were male 1.52; Female 1. 2) Age distributions were 300 cases (44.2%) in 3rd decade, 189 cases (27.8%) in 2nd decade, 146 cases (21.5%) in 4th decade in order. 3) Site distributions were 336 cases (49.5%) in left, 343 cases (50.5%) in right. 4) Durations of the disease were 233 cases (34.3%) from 6 year to 10 year, 132 cases (19.4%) from 11 year to 15 year, 114 cases (6.8%) from 16 year to 20 year in order. 5) Perforation sites of the drum were 319 cases (47.0%) in total perforation, 159 cases (23.4%) in large perforation, 110 cases (16.2%) in small or moderate perforation in order. 6) X-ray findings of temporal bone were 443 cases (65.2%) in sclerotic, 174 cases (25.6%) in bony destruction and cholesteatoma. 7) Routes of approach were 615 cases (90.6%) in endaural incision, 62 cases (9.2%) in postaural incision. 8) Conditions of ossicles were 426 cases (62.7%) in destruction, 253 cases (37.33%) in intact. 9) Graft materials of the drum were fascia 312 cases (63.2%), cartilage 182 cases (36.8%). 10) Types of operation were 224 cases (33.0%) in mastoidectomy with tympanoplasty; Type I was 57 cases (25.4%), Type II was 36 cases (16.1%), Type III was 92 cases (41.1%), Type IV was 39 cases (17.4%), 157 cases (23.1%) in radical mastoidectomy, 156 cases (23.0%) in tympanoplasty without mastoidectomy; Type I was 94 cases (60.3%), Type II was 28 cases (17.9%), Type III was 27 cases (17.3%), Type IV was 7 cases (4.5%), 96 cases (14.1%) in myringoplasty, 20 cases (3.0%) in revision: Mastoid was 2 cases (10.0%), Tympanoplasty Type I was 2 cases (10.0%), Tympanoplasty Type II was 6 cases (30.0%), Type III was 5 cases (20.0%), Type IV was 5 cases (25.5%), 19 cases (2.8%) in stage operation in order.
