Clinical analysis of contact Nd:YAG laser tonsillectomy.

ABSTRACT

The tonsillectomy is one of the most common practices in the otolaryngologic operative procedure. This operation is sometimes related to the unexpected bleeding and can be troubled surgeries, and to resolve these problems, cryosurgery and argon laser surgery previously were introduced, but which were not established as clinical method of tonsillectomy. New developed Nd : YAG laser system by a contact tip which is made by new synthetic sapphires, is used to focus the beam on the target spot. The present study concerns the new clinical applications in tonsillectomy. The author analysed 90 patients who underwent contact Nd : YAG laser tonsillectomy comparision with 82 patients who underwent standard dissection from June 1991 to December 1991 in Chung-Ang University Hospital, to evaluate for usability of laser tonsillectomy. The results obtained were as follows : 1) In comparision with standard technique, laser tonsillectomy had shorter operation time, smaller intraoperative blood loss and delayed wound healing. 2) None of postoperative pain, bleeding and vomiting were statistically significant. 3) The mean depth of necrotic cell layer was 22.2μm(S.D.=5.5μm) in 40 tonsils which were extirpated by contact Nd : YAG laser tonsillectomy. From the above results, it is thought that the tonsillectomy with contact Nd : YAG laser technique is clinically beneficial as one of the operative treatment for chronic tonsillitis and patients who had been with peritonsillar abscess, and it is useful to operate on patients with bleeding disorders and older patients.
