ABSTRACT

Epistaxis is such a common problem that every otolaryngologist should be prepared to handle it. Most of cases are controlled with conservative methods, but there are occasional cases that need invasive treatment. The ligation of internal maxillary artery cannot be considered in those patients who would be poor surgical candidates. A markedly small antrum may make this porcedure extremely difficult. The advantages of angiographic embolization include the avoidance of surgery and general anesthesia, the identification of the site of hemorrhage, and an efficiency that is equal to, or greater than that of ligation. The procedure may be repeated if collateralization occurs and may be employed whether the bleeding is hypertensive, posttraumatic, postsurgical, or due to an underlying tumor or vascular malformation. The authors report three cases of uncontrollable epistaxis in which angiographic localization of the site of hemorrhage as well as successful therapeutic embolization with polyvinyl alcohol(Ivalon) were performed.
