The Significance of Muscular and Mucociliary Activity Test of Eustachian Tube in Chronic Otitis Media

ABSTRACT

Among eustachian tube function tests, there are Valsalva maneuver, Toynbee method, Politz method, and eustachian tube catheterization methods as ventilatory function test. Although inflation-deflation test has been widely used in clinical practice, the test of muscular and mucociliary activity affecting in the transport of secretion is also considered to be important. Therefore, in order to determine the clinical significance of muscular and mucociliary activity, I performed muscular and mucociliary activity test by using saccharine solution and powder on 40 cases with chronic otitis media who underwent middle ear surgery and 14 cases with traumatic ear drum perforation as control group. The results obtained are as follows ; 1) The average clearance time of muscular and mucociliary activity test in 14 cases of traumatic ear drum perforation was 5.50±2.44 minutes and 11.32±4.25 minutes respectively. Also in cases of chronic otitis media with good tubal function, the average clearance time in 14 cases of muscular activity test and 9 cases of mucociliary activity test was 4.21±2.83 minutes and 9.00±4.46 minutes respectively. 2) The number of ears with eustachian tubal function in muscular activity test, mucociliary activity test, inflation test, and deflation test were 22, 14, 11 and 8 respectively, and those with good eustachian tubal function and normal tympanic orifice were 14(63.6%), 9(64.3%), 5(45.4%) and 5(62.5%) respectively. Although the number of ears were higher in muscular activity test, the predictability of normal tympanic orifice was significant between each test statistically. Even though the number of ears are not enough, I summarized that muscular and mucociliary activity test are to be recommended as a preoperative evaluation procedure of eustachian tube function as well as inflation-deflation test which has been generally accepted.
