The Clinical Evaluation of Reoporation in Recurrent Sinusitis

ABSTRACT

As the surgical treatment of chronic sinusitis, conservative or radical surgery was used, but good results were not accomplished for a long time. Kitamura said that problems from the operation, the pathologic states of nasal cavity and paranasal sinus and postoperative care were the one of recurrent sinuse. Recently, allergic reaction were said to be related to recurrence and the surgical therapy was used more than medical therapy in order to treat the inflammatory diseases of paranasal sinuses. For this reason, the author studied the 29 cases of recurrent sinusitis and analyzed the cause of recurrence, postoperative healing process and reoperative findings to improve the postoperative results and the surgical procedure. The results of this study were as follows : 1) The recurrence of the symptoms mostly occurred within 5 years after operation and affect male between the ages of 30 and 40 years. 2) All cases except postoperative cheek cyst and were filled with edematuous mucosa and granulation tissues. 3) The findings of nasal cavity showed the hypertrophy of inferior turbinates and the polypoid change of middle turbinates and also showed the deviation of nasal septum. 4) All cases showed the dysfunction of ventilation and drainage due to the obstruction of middle meatus, dysfunction in drainage between the maxillary and ethmoid sinus and the obstruction of counter opening. 5) All symptoms were usually improved after reoperation. 6) In the chronic sinusitis, the pathologic lesion of nasal cavity and paranasal sinus was operated at the same time. 7) The first operation should be performed carefully for the mucosal remnants and granulation tissue was tend not to totally remove in operation.
